Sports Physicals, also known as a pre-participation physical exam (PPE), is a physical exam, which can help to ensure that you or your child is participating in their sport, or activity, safely. It can also help to identify health problems that may require further evaluation prior to participating in their sport or activity safely.
Before the exam

Schedule an appointment by selecting BOOK NOW . If you would prefer a different time than what is available, please call us so we can get that set up for you!

Complete the forms required by the organization requesting the physical.

Plan to bring the completed forms* to your appointment for the provider to review and complete

If the person being examined wears contacts or glasses, bring those too.

Complete the consent form (link will be sent to you or you can access it at the bottom of this page under helpful links)
*Forms and consent need to be provided by parent/guardian if patient is a minor
*Minors must be accompanied by parent/guardian
During the exam
All exams must be done in-person with the provider
During the physical exam, our board-certified nurse practitioner:

Review the patient’s medical history

Conduct a physical examination

Complete forms* needed for participation, if qualified
*Forms and consent need to be provided by parent/guardian if patient is a minor
Cost - $25 per exam
PPE exams are not typically covered by insurance. Rise Up Health does not process claims through insurance. Payments are accepted in the forms of cash, credit card (including HSA & FSA), or Venmo.
